Instructions
Please use the Corrective Action Response (CAR) form to record your corrective action response for each nonconformance listed in the Current Nonconformance worksheet of your current Company Quality Audit report.
There are two CAR forms available:
- The first form can accommodate two corrective action responses.
- The second form can be used for a single response in the event that the response requires more space.
Multiple forms can be submitted as separate PDF files to PCI for review.
How to Complete the Forms
- Please note that the nonconformance number must match the number assigned in the Current Nonconformance worksheet of the latest audit report.
- The auditor’s description of the nonconformance should be briefly summarized; it does not need to be restated in its entirety.
The bottom portion of the form, titled “Erector’s Corrective Action Response (CAR),” must include the following:

A brief summary of the root-cause analysis, which must identify the underlying problem that caused the nonconformance to occur

The corrective/preventive actions taken to mitigate the issues caused by the underlying problem and to support the potential elimination of the problem
Please note that objective evidence for each recurring major nonconformance must be included with your CAR submission. This objective evidence may include meeting minutes, new or modified training or standard procedures, or photos of corrected actions, and so on. Reminder: Completed CAR forms are due to PCI within 30 days of receiving the audit report.
